System of Care

While a variety of services in the areas of mental health and addiction are in northeast Indiana, they are not well-connected, and thus can be difficult for individuals needing care to navigate.

In 2016, The Lutheran Foundation hired two new employees to serve as System of Care Coordinators to help fill in those gaps. The initial funding for these positions was provided by grants from the State of Indiana’s Division of Mental Health and Addiction (DMHA).  In 2017, DMHA awarded The Lutheran Foundation three grants, two of which were to continue the system of care work already in process, and an additional grant aimed on growing system of care in four northern counties.
